Original birth certificate issued by the Quebec civil registry (issued less than 5 years ago and including parental information), or, if in another province or territory of Canada, birth certificate including parental information issued after 1994.
Original divorce or widowhood certificate (if applicable).
Valid Canadian passport (photo and signature pages)
Proof of address in Canada
